| Student Athlete Forms Each new student athlete must complete the following forms to be eligible for intercollegiate athletics participation at Oklahoma Baptist University. Physical This form is to be used for student-athletes who miss the on campus physical. Medical History This form asks for information regarding medical history. HIPPA This form is required for the release of medical information. Insurance Information This form is required for treatment of any athletic injuries. Student-Athlete Handbook This handbook contains guidelines for participation in athletics at Oklahoma Baptist University. Student-athletes will receive a copy of this handbook on the day of physicals. Forms at the end of the handbook must be signed and returned to OBU. Assumption of Risk This form must be signed by parents indicating that they aknowledge the risk of intercollegiate athletics. Authorization for Medical Treatment This form is required to give permission for treatment of any athletic injuries. Transfer Student-Athlete All transfer student athletes must have medical records from previous colleges forwarded to the OBU Athletic Training Department prior to arrival. |
